People were thrown from a catapult into a net at Middlemoor Water Park in Fiddington, Somerset.
The ride was shut down after Kostadin Yankov, 19, was thrown outside the net and died on 24 November 2002.

This is very old footage.
The footage is apparently more than four years old and was taken in Somerset, England, before a fatal accident (after which the operators reportedly dismanteld the operation).

Also, after a cursory googling for:
"Kostadin Yankov" catapult
... it seems like the operators had become sloppy in the time leading up to Yankov's death: there were reports of other participants landing off-center of the net, near the edge.
