1. He's a nerd and we nerds are known for our social ineptitude
2. It is clear that whatever it was that was in his teeth was clearly bothering him so he waited for his chance and BAM! picked it.
Unrelated, but it seems that half the voices Billy does causes him some level of physical pain (i.e., Ren)
