I remember watching a completely serious safety video at one of my previous jobs where a guy is killed by a flying high pressure cylinder while he's eating lunch in another room. The top gets knocked off, it goes flying through a window, and hits him in the head.
I'm not sure what the message was supposed to be for that one. "Never let your guard down, even during lunch, in a completely different room."
